N-terminal pro-brain natriuretic peptide (NT-proBNP)

Target
NT-proBNP
Molecular classification
Other (cleaved peptide fragment, cardiac hormone precursor)
01

Overview

N-terminal pro-brain natriuretic peptide (NT-proBNP) is a non-active peptide fragment derived by cleavage of proBNP in cardiomyocytes in response to myocardial wall stress. While NT-proBNP itself does not exert biological effects, its concentration in plasma correlates closely with cardiac function, and it is extensively used in clinical practice as a biomarker for heart failure diagnosis, prognosis, and monitoring, as well as risk stratification for cardiovascular and some cerebrovascular events. Clearance is primarily renal, and levels are affected by kidney function, age, and other systemic conditions.

02

Mechanism of action

Not applicable; NT-proBNP is not a direct drug target. Its levels reflect cardiac wall stress and neurohormonal activity.

03

Biological functions

Biomarker for myocardial stressIndicator of heart failure severityPrognostic indicator for cardiovascular and cerebrovascular risk
04

Disease associations

Cardiovascular disease (especially heart failure, coronary artery disease, aortic stenosis)Cerebrovascular events (stroke, abnormal brain aging)
05

Safety considerations

Renal impairment (clearance of NT-proBNP is reduced, leading to elevated levels irrespective of cardiac function)Non-specific elevation (levels may rise with age, renal failure, anemia, and other non-cardiac conditions)Not a direct therapeutic target, so no drug-specific toxicities
