Overview

Naïve T cell depletion is a transplantation strategy in which only the naïve T cell subset—identified by expression of CD45RA—is selectively removed from donor hematopoietic cell grafts to prevent severe graft-versus-host disease (GVHD) while sparing memory T cells that provide protective immunity and maintain the graft-versus-leukemia (GVL) effect. This approach utilizes immunomagnetic beads or other selection technologies to deplete CD45RA-positive T cells before transplantation. Clinical studies indicate this method dramatically reduces chronic GVHD rates compared to pan–T cell depletion, preserves anti-infective and anti-tumor responses, and may improve post-transplant morbidity and non-relapse mortality outcomes. In summary, "Naïve T cell depletion" should not be coded as a therapeutic target molecule or receptor, but rather recognized as a clinical manipulation of cellular populations for transplant benefit.
