Overview

NADH-ubiquinone oxidoreductase chain 4 (ND4) is a core, mitochondrially encoded subunit of Complex I, the largest enzyme in the mitochondrial respiratory chain [2, 5]. It is essential for the catalytic activity of the complex, facilitating the transfer of electrons from NADH to ubiquinone and the translocation of protons across the inner mitochondrial membrane to drive ATP synthesis [1, 5]. Mutations in the MT-ND4 gene, particularly the G11778A point mutation, are the primary cause of Leber hereditary optic neuropathy (LHON), a rare genetic disorder leading to rapid, bilateral vision loss due to retinal ganglion cell death [1, 10]. Because ND4 is encoded by mitochondrial DNA, it is a unique therapeutic target for allotropic gene therapies like lenadogene nolparvovec and NR082, which deliver a nuclear-encoded version of the gene to restore mitochondrial function [3, 10]. Clinical management also involves mutation-independent therapies like idebenone, which aim to bypass the defective complex [4]. Safety concerns for ND4-targeted therapies include ocular inflammation, AAV vector immunogenicity, and the challenges of delivering genetic material into the mitochondrial environment [3, 9].
