Overview

This target represents a complex of capsular polysaccharides derived from Neisseria meningitidis (serogroups A, C, W-135, and Y) covalently bound to a diphtheria toxoid carrier protein (FDA, 2020). Neisseria meningitidis is a primary cause of life-threatening bacterial meningitis and septicemia, often progressing rapidly in healthy individuals (CDC, 2023). While bacterial polysaccharides are typically T-cell independent antigens that do not induce long-term memory or protect infants, conjugation to a protein carrier like diphtheria toxoid or its non-toxic mutant CRM197 converts them into T-cell dependent antigens (StatPearls, 2023). This conversion allows for the recruitment of T-helper cells, which facilitates B-cell differentiation into memory cells and the production of high-affinity IgG antibodies (PubMed, PMID: 21939495). Vaccines targeting this complex, such as Menactra and Menveo, are used to provide durable protection against invasive meningococcal disease in various age groups, particularly adolescents and those at increased risk (NIH, 2022).

Mechanism of action

The conjugation of bacterial capsular polysaccharides to a protein carrier (diphtheria toxoid or CRM197) converts the T-cell independent polysaccharide antigens into T-cell dependent antigens, enabling recruitment of T-helper cells, high-affinity antibody production, and long-term immune memory.
