The Nerve to vastus lateralis muscle (NVL) is a major motor branch of the femoral nerve, arising from the L2 through L4 spinal nerve roots. Its primary biological function is to provide the essential motor innervation required for the contraction of the vastus lateralis, the largest muscle of the quadriceps femoris group, which facilitates knee extension and joint stability. Anatomically, the NVL often follows a predictable course alongside the descending branch of the lateral circumflex femoral artery, typically dividing into multiple terminal branches before entering the muscle belly. While it is an anatomical structure rather than a molecular receptor, the NVL is a significant clinical target for regional anesthesia and ultrasound-guided nerve blocks, particularly in the management of postoperative pain following knee or hip arthroplasty. Selective blockade of this nerve is frequently studied for its potential to provide targeted analgesia while sparing more significant motor functions compared to a proximal femoral nerve block. However, iatrogenic injury to the NVL during surgical procedures, such as anterolateral thigh flap harvests or femoral fracture repairs, can lead to muscle atrophy and significant functional impairment in lower limb mobility.
