NY-ESO-1, encoded by the CTAG1B gene, is a cancer-testis antigen (CTA) characterized by its restricted expression in normal adult tissues, primarily the testis and placenta, and its aberrant expression in various cancers, such as synovial sarcoma and melanoma (NCI Drug Dictionary). The 157-165 amino acid sequence (SLLMWITQC) is a highly immunogenic epitope that is processed and presented on the cell surface by the HLA-A*02:01 allele (PubMed ID: 9491742). This complex is recognized by specific T-cell receptors (TCRs), making it a primary target for TCR-engineered T-cell (TCR-T) therapies and bispecific T-cell engagers (Adaptimmune Therapeutics, 2024). Drugs like afamitresgene autoleucel have been developed to target this pMHC, demonstrating significant clinical efficacy in treating NY-ESO-1-positive, HLA-A*02:01-positive tumors (PubMed ID: 38537777). The high tumor specificity of NY-ESO-1 minimizes the risk of off-target toxicity, although challenges such as cytokine release syndrome and HLA downregulation remain (PubMed ID: 26034009).
