Overview

New York esophageal squamous cell carcinoma 1 (NY-ESO-1) is a prominent member of the cancer-testis antigen (CTA) family, encoded by the CTAG1B gene. Under physiological conditions, its expression is strictly limited to germ cells in the testis and occasionally the placenta, which lack MHC Class I expression, making it an exceptionally specific target for immunotherapy. In various malignancies, including synovial sarcoma and melanoma, NY-ESO-1 is aberrantly re-expressed and its degraded peptides are presented on the cell surface by Major Histocompatibility Complex (MHC) molecules, particularly HLA-A*02. This presentation allows the immune system to recognize intracellular oncogenic proteins that are otherwise inaccessible to traditional antibody-based therapies. Current therapeutic approaches focus on TCR-engineered T-cell therapies (TCR-T) and vaccines designed to prime the immune system to identify and eliminate NY-ESO-1-positive tumor cells. While highly effective in specific subsets of patients, the requirement for specific HLA matching and the potential for tumor immune evasion via antigen loss remain significant clinical considerations.
