Overview

NHERF family PDZ scaffold protein 4 (NHERF4) is an epithelial-enriched scaffolding protein belonging to the NHERF family, characterized by multiple PDZ domains that enable protein–protein interactions, particularly at the apical membrane of epithelial cells[1][3]. NHERF4 is the most divergent member of the NHERF family and contains four PDZ domains, setting it apart structurally from NHERF1 and NHERF2, which have two PDZ domains each[1]. It organizes and regulates multiprotein complexes involved in epithelial cell polarity, ion transport (such as interaction with the type IIa sodium–phosphate co-transporter), and second messenger signaling[1][3]. Emerging studies suggest unique binding partners and subcellular localization for NHERF4 compared to other family members, but its precise biological functions require further investigation[1][3][5]. There are no established therapeutic drugs or mechanisms of action targeting NHERF4, nor are there recognized safety concerns or biomarkers. Diseases associated with NHERF4 mutations include motility-related diarrhea and lissencephaly 9[4].
