Non-specific cellular and tissue milieu

Non-specific cellular and tissue milieu refers to the general physiological environment, including the extracellular fluid, cytoplasm, and lipid membranes, that serves as a site of action for certain pharmacological agents (StatPearls, 2023). Unlike traditional drug targets such as receptors, enzymes, or transporters, this milieu does not involve specific high-affinity binding sites (Goodman & Gilman's The Pharmacological Basis of Therapeutics). Instead, drugs acting on the milieu typically produce therapeutic effects through broad physical or chemical changes, such as altering the pH of gastric acid, increasing the osmotic pressure in the renal tubules, or adsorbing toxins in the intestinal lumen (NCBI Bookshelf, 2022). Common examples of agents targeting this milieu include antacids, osmotic diuretics like mannitol, and adsorbents like activated charcoal. While these interactions are clinically useful, they are often characterized by a lack of molecular specificity, which can lead to systemic physiological disturbances like electrolyte imbalances if not carefully monitored (PubMed, PMC7235931). Consequently, this term is often used in drug classification to describe mechanisms that fall outside the scope of specific macromolecular targeting.
