Overview

Non-specific ionic substrates refer to a category of therapeutic targets that are not defined by a specific protein, such as a receptor or enzyme, but rather by inorganic ions or the chemical environment of physiological fluids (StatPearls, 2023). This classification typically includes hydrogen ions (protons) neutralized by antacids, as well as potassium or phosphate ions sequestered by ion-exchange resins in the gastrointestinal tract (FDA, 2015; PubMed, 1999). Because these interactions are governed by physical and chemical principles like ion exchange, chelation, or osmotic gradients, they lack the high specificity associated with traditional drug-protein binding (Goodman & Gilman's, 2018). Clinically, agents targeting these substrates are used to manage conditions such as hyperkalemia, hyperphosphatemia, and gastric acidity (NIH, 2022). While these substrates are essential for physiological homeostasis, they do not constitute a single biological entity or a distinct molecular target. Consequently, this term serves as a functional grouping for non-receptor-mediated pharmacological actions.
