Overview

Non-specific phagocytic uptake by the reticuloendothelial system (RES), now more commonly referred to as the mononuclear phagocyte system (MPS), is a physiological process where specialized macrophages in the liver (Kupffer cells), spleen, and lymph nodes identify and remove foreign particulate matter from the circulation. This process is primarily mediated by opsonization, where plasma proteins like immunoglobulins and complement factors coat the substance, signaling macrophages to perform phagocytosis. While essential for clearing pathogens and cellular debris, this mechanism represents a significant hurdle in pharmacology, particularly for nanomedicines and imaging agents, as it often leads to rapid blood clearance and off-target accumulation in the liver and spleen. Strategies such as PEGylation are frequently employed to 'cloak' drugs from this system to extend their therapeutic half-life. In diagnostic medicine, however, this uptake is intentionally exploited using radiopharmaceuticals like Technetium-99m sulfur colloid to visualize the functional integrity of the liver, spleen, and lymphatic system.

Mechanism of action

Non-specific uptake occurs via opsonization, where plasma proteins (opsonins) coat foreign particles, followed by recognition and engulfment by macrophage receptors such as Fc receptors, complement receptors, and scavenger receptors.
