Non-transferrin-bound iron (NTBI) and the labile iron pool (LIP) represent the fraction of iron that is not sequestered by high-affinity proteins like transferrin or ferritin (PMID: 15607119). These iron species are redox-active and can catalyze the formation of reactive oxygen species (ROS) via the Fenton reaction, leading to oxidative damage to cellular membranes, proteins, and DNA (PMID: 23841501). In clinical conditions of iron overload, such as hereditary hemochromatosis or transfusion-dependent thalassemias, NTBI levels rise significantly and contribute to organ damage in the liver, heart, and endocrine glands (NIH: Hemochromatosis). The LIP also plays a central role in ferroptosis, an iron-dependent form of regulated cell death characterized by lipid peroxidation (PMID: 32203312). Therapeutic management involves the use of iron chelators, such as deferoxamine, deferiprone, and deferasirox, which bind these free iron species to neutralize their toxicity and promote excretion (StatPearls: Iron Toxicity). Monitoring these pools is essential for assessing the risk of iron-mediated toxicity and the efficacy of chelation therapy.
Iron chelators bind to free ferric iron (Fe3+) or loosely bound iron in the NTBI and LIP to form stable, chemically inert complexes that are subsequently excreted via the kidneys or bile, thereby preventing the iron from participating in redox reactions that generate toxic free radicals (PMID: 15607119; StatPearls: Iron Toxicity).
