Overview

"Normal gut microbiota restoration" refers to therapeutic interventions aimed at reestablishing a diverse and healthy microbial ecosystem within the gastrointestinal tract, typically in patients with dysbiosis (an imbalance or alteration in the gut microbial community). Restoration can be achieved using probiotics (live beneficial microorganisms), prebiotics (dietary substrates supporting beneficial microbes), and fecal microbiota transplantation (FMT), which transfers stool (and its microbes) from a healthy donor to the patient. The goal is to restore biological functions essential to host health, including protection against pathogens, immunomodulation, metabolic homeostasis, and gut–brain communication. These therapies are proven for certain infections (Clostridioides difficile) and are investigational or under study for other gastrointestinal, metabolic, and neurodegenerative disorders. Unlike conventional drug targets, microbiota restoration addresses an entire ecosystem, not an individual molecular structure. Safety concerns center on infection risk, immune compatibility, and variable efficacy due to individual microbiome composition.

Mechanism of action

Repopulation and diversification of beneficial microorganisms in the gut; Displacement and inhibition of pathogenic microorganisms; Restoration of metabolite profiles (e.g., increase in short-chain fatty acids: butyrate, propionate, acetate); Modulation of immune responses and reduction of inflammation; Re-establishment of barrier function and homeostasis

Safety considerations

Transmission of infectious agents with donor materialAllergic or immune reactionsVariability in efficacy and colonization due to patient-specific factors (age, genetics, baseline microbiome, severity of underlying disease)Lack of long-term safety data for some microbiota restoration therapies
