Overview

The normal vascular endothelium and extracellular matrix (ECM) represent the fundamental structural and functional units of the circulatory system. The endothelium is a specialized monolayer of cells that acts as a semi-permeable barrier, regulating the exchange of fluids, nutrients, and cells between the blood and surrounding tissues while maintaining vascular tone and hemostasis (StatPearls, 2023). The ECM provides the essential physical scaffold and biochemical signaling environment, composed of proteins like collagen, elastin, and laminin, which support cellular adhesion and tissue integrity (NCBI, 2022). While not a single molecular target, this complex is a critical focus in pharmacology because it serves as the primary barrier for drug delivery and the site of action for various anticoagulants and anti-angiogenic therapies (PubMed, 2021). Pathological alterations in these structures are central to the progression of atherosclerosis, hypertension, and tumor-associated vascular leakage. Consequently, maintaining the health of the normal endothelium and ECM is a major therapeutic goal in preventing cardiovascular and inflammatory diseases.
