The nucleus pulposus is the gelatinous inner core of the intervertebral disc, characterized by a high concentration of water (70-90%) and a specialized extracellular matrix rich in proteoglycans like aggrecan and type II collagen (StatPearls, 2023). Its primary biological role is to provide hydrostatic pressure to resist axial compression and maintain the height and flexibility of the spinal column (PubMed, 2021). Disc water content is a fundamental physiological parameter that reflects the health and functional integrity of the disc; a significant reduction in hydration is a hallmark of intervertebral disc degeneration (IDD) and is often associated with chronic low back pain (NIH, 2022). While the nucleus pulposus and its water content are not molecular targets in the traditional sense, they represent the primary site for regenerative therapies, including the delivery of hydrogels, growth factors, and cell-based treatments (PubMed, 2020). These interventions aim to restore the osmotic environment and structural properties of the tissue to alleviate pain and improve spinal mobility.
Therapeutic approaches typically involve the restoration of osmotic pressure and extracellular matrix integrity through the delivery of proteoglycan-mimetic hydrogels or growth factors that stimulate endogenous matrix production (PubMed, 2021).
