Overview

The ocular surface epithelial glycocalyx is a specialized extracellular matrix layer composed primarily of membrane-associated mucins (MUC1, MUC4, and MUC16) and glycoproteins that coat the apical surface of the corneal and conjunctival epithelium (Gipson, 2011, PMID: 21705598). This structure serves as a critical interface between the ocular surface and the tear film, providing lubrication, maintaining hydration, and acting as a physical and chemical barrier against pathogens and environmental insults (Mantelli & Argüeso, 2008, PMID: 18435583). In pathological conditions such as dry eye disease, the glycocalyx is often degraded or diminished, leading to increased friction, inflammation, and epithelial damage (Uchino et al., 2013, PMID: 23554833). Therapeutic strategies aim to restore this layer through the use of mucin secretagogues, such as rebamipide and diquafosol, or by applying mucomimetic agents like hyaluronic acid that stabilize the tear film (Kinoshita et al., 2013, PMID: 23307496). Understanding the glycocalyx is essential for developing treatments that address the underlying causes of ocular surface instability and discomfort.
