The ocular surface mucin-water matrix is a specialized physiological barrier located at the interface of the corneal and conjunctival epithelium and the tear film (Mantelli & Argüeso, 2008). It is composed of membrane-associated mucins, such as MUC1, MUC4, and MUC16, which form the glycocalyx, and secreted mucins like MUC5AC that are suspended within the aqueous layer (Shimazaki, 2018). This matrix serves a critical role in maintaining ocular health by providing lubrication, ensuring uniform wetting of the eye, and acting as a physical and chemical barrier against pathogens and environmental debris (Mantelli & Argüeso, 2008). In patients with dry eye disease, the density and quality of this matrix are often diminished, leading to tear film instability and inflammation (Koh, 2015). Therapeutic agents like Rebamipide and Diquafosol target this matrix by enhancing the expression and secretion of mucins from epithelial and goblet cells (Urashima et al., 2012; Koh, 2015). By restoring the integrity of the mucin-water matrix, these treatments improve tear film stability and alleviate the symptoms of ocular surface disorders.
