Overview

The ondansetron–dexamethasone admixture is a clinical combination of two pharmacological agents, ondansetron and dexamethasone, used for the prevention of nausea and vomiting associated with emetogenic chemotherapy and surgery (StatPearls, 2023). Ondansetron is a competitive 5-HT3 receptor antagonist that blocks serotonin signaling in the peripheral and central nervous systems (PubChem, CID 4595). Dexamethasone is a synthetic glucocorticoid that acts on the glucocorticoid receptor to exert anti-inflammatory and antiemetic effects, though its precise mechanism in emesis is not fully elucidated (StatPearls, 2023). The co-administration of these agents provides a synergistic effect, significantly improving the complete response rate in patients compared to monotherapy (PubMed, PMID 11033133). Because these drugs are often mixed in intravenous bags or syringes, their physical and chemical compatibility is a critical consideration for healthcare providers to ensure stability and potency (PubMed, PMID 8832930).

Mechanism of action

Ondansetron is a selective 5-HT3 receptor antagonist that prevents serotonin from binding to receptors on vagal afferents and in the central nervous system (StatPearls, 2023). Dexamethasone is a glucocorticoid receptor agonist that modulates gene transcription to reduce inflammation and potentially inhibit prostaglandin synthesis in the brain, which contributes to its antiemetic synergy (StatPearls, 2023).
