Overview

The opportunistic intestinal pathogens and microbiota ecosystem refers to the complex community of microorganisms residing in the human gastrointestinal tract and the specific pathogenic species that exploit disruptions in this community. The gut microbiota provides essential functions including nutrient metabolism, immune system modulation, and protection against pathogen colonization through a process known as colonization resistance (NIH). When this ecosystem is disturbed—often by broad-spectrum antibiotic use—opportunistic pathogens such as Clostridioides difficile or Vancomycin-resistant Enterococci (VRE) can proliferate, leading to severe enteric infections and systemic inflammation (PubMed). Therapeutic strategies targeting this ecosystem focus on either the eradication of specific pathogens using antibiotics or the restoration of a healthy microbial balance through probiotics and fecal microbiota transplantation (FMT) (FDA). This target is considered incorrect in a classical pharmacological sense because it represents a multi-organism ecological system rather than a single protein, receptor, or enzyme.
