The optic nerve head (ONH) vasculature is a specialized microcirculatory network responsible for supplying oxygen and nutrients to the retinal ganglion cell axons as they transition from the eye to the brain. This vascular system is complex, receiving blood from the central retinal artery, the short posterior ciliary arteries, and the circle of Zinn-Haller (StatPearls, 2023). Its primary biological role is to maintain metabolic homeostasis through robust autoregulation, ensuring stable blood flow despite fluctuations in intraocular pressure (IOP) or systemic blood pressure (PubMed, 2021). Dysregulation or insufficiency of this vasculature is a hallmark of glaucoma and ischemic optic neuropathies, where reduced perfusion leads to axonal degeneration (NIH, 2022). While not a single molecular target, the ONH vasculature is a critical therapeutic focus; medications like dorzolamide and netarsudil are studied for their ability to enhance blood flow through direct vasodilatory effects or by improving the pressure gradient via IOP reduction (Journal of Ocular Pharmacology, 2020). Clinical assessment of this system has been revolutionized by Optical Coherence Tomography Angiography (OCTA), which allows for non-invasive monitoring of vessel density as a biomarker for disease progression (Investigative Ophthalmology & Visual Science, 2019). Understanding the physiological and pathological states of the ONH vasculature is essential for developing neuroprotective strategies in ophthalmology.
Drugs interact with the optic nerve head vasculature primarily by reducing intraocular pressure (IOP) to increase ocular perfusion pressure (OPP) or by inducing direct vasodilation of microvessels through carbonic anhydrase inhibition, calcium channel blockade, or Rho-kinase inhibition to improve blood flow.
