The oral microbiota producing volatile sulfur compounds (VSCs) consists of a diverse group of predominantly anaerobic Gram-negative bacteria that reside on the tongue dorsum and within periodontal pockets. These microorganisms, including species such as Porphyromonas gingivalis, Treponema denticola, and Fusobacterium nucleatum, utilize proteolytic enzymes to degrade sulfur-containing amino acids like cysteine and methionine into malodorous gases, primarily hydrogen sulfide and methyl mercaptan (PubMed: 24571068, 16530701). These VSCs are the primary etiological agents of intra-oral halitosis and have been implicated in the pathogenesis of periodontal disease by increasing the permeability of the oral mucosa and promoting pro-inflammatory cytokine release (PubMed: 15752094). Therapeutic strategies targeting this microbiota include the use of broad-spectrum antimicrobials like chlorhexidine to reduce bacterial load, zinc salts to chemically neutralize VSCs, and probiotics to restore a non-malodorous microbial balance (PubMed: 21287871). While effective for symptom management, long-term use of certain chemical agents can lead to side effects such as tooth staining or shifts in the natural oral flora.
