Overview

The oral mucosa and gingival tissue surface constitute the protective lining of the oral cavity, acting as a vital barrier between the external environment and the underlying tissues. This anatomical structure is composed of stratified squamous epithelium, which may be keratinized (as in the hard palate and gingiva) or non-keratinized (as in the cheeks and floor of the mouth), supported by a connective tissue layer known as the lamina propria (StatPearls, 2023). Its primary biological functions include protection against mechanical forces, prevention of pathogen entry, and sensory perception (NIH, 2023). While not a single molecular target, it is a critical site for the administration of local anesthetics, anti-inflammatory agents, and systemic drugs via transmucosal routes to avoid first-pass metabolism (Hearnden et al., 2012). Pathological conditions affecting this surface include gingivitis, periodontitis, and various forms of oral cancer, often characterized by the breakdown of the epithelial barrier and chronic inflammation (NIDCR, 2023). Therapeutic interventions targeting this area frequently involve mucoadhesive formulations designed to prolong drug contact time with the tissue surface (Sudhakar et al., 2006).

Mechanism of action

Drugs interact with the oral mucosa and gingival surface through topical absorption, where they either exert local effects (e.g., local anesthesia, anti-inflammatory action) or penetrate the epithelial barrier to enter the systemic circulation via the dense capillary network in the lamina propria, bypassing first-pass hepatic metabolism (Hearnden et al., 2012; StatPearls, 2023).
