Overview

The osmotic gradient for peritoneal fluid removal describes the difference in osmolality (concentration of solutes, often using glucose or other osmotic agents) between blood plasma and peritoneal dialysis fluid. This gradient is harnessed in peritoneal dialysis to draw water out of the bloodstream, across the peritoneal membrane, and into the dialysis cavity by osmosis. The efficiency and amount of fluid removed depend on the steepness of this gradient, the characteristics of the peritoneal membrane, and the properties of the osmotic agent used (primarily glucose, but also icodextrin or amino acids as alternatives). This process is a cornerstone of fluid management in patients with renal failure receiving peritoneal dialysis, but it is not a druggable molecular target or a discrete biological entity.
