Overview

Osteogenic markers are a diverse group of proteins, enzymes, and transcription factors that serve as indicators of osteoblast activity, differentiation, and bone formation (StatPearls, 2023). These markers, which include bone-specific alkaline phosphatase (BAP), osteocalcin, and procollagen type I N-terminal propeptide (P1NP), are essential for the synthesis and mineralization of the bone extracellular matrix (PubMed, 2018). In clinical and research settings, they are used to monitor bone turnover rates and the efficacy of anabolic or anti-resorptive therapies (NIH, 2022). While not a single therapeutic target themselves, they represent the downstream effects of drugs targeting bone metabolism, such as parathyroid hormone analogs (e.g., teriparatide) or sclerostin inhibitors (e.g., romosozumab) (FDA, 2019). Changes in these markers provide critical insights into the progression of metabolic bone diseases like osteoporosis, Paget's disease, and bone metastasis (Journal of Bone and Mineral Research, 2020). Mechanism of action

Therapeutic agents modulate bone remodeling by either stimulating osteoblast activity (anabolic) or inhibiting osteoclast activity (anti-resorptive), which is reflected by changes in the expression or serum levels of these markers (StatPearls, 2023; PubMed, 2018).
