Overview

Paired-like homeobox 2B (PHOX2B) is a master regulator transcription factor essential for the development of the autonomic nervous system, particularly the noradrenergic lineage. It coordinates the differentiation of sympathetic, parasympathetic, and enteric neurons, as well as the formation of the retrotrapezoid nucleus involved in chemoreception and breathing regulation (UniProt P54871). Clinically, PHOX2B is the defining gene for Congenital Central Hypoventilation Syndrome (CCHS), where mutations lead to life-threatening respiratory failure during sleep (PubMed 12652297). In the field of oncology, PHOX2B is a highly specific marker for neuroblastoma, as its expression is largely restricted to the embryonal sympathetic nervous system and absent in most adult tissues (PubMed 15520197). This high specificity has made it a prime candidate for minimal residual disease monitoring and novel immunotherapeutic approaches. Recent advancements include the development of CAR-T cells and TCR-like antibodies that target PHOX2B-derived peptides presented on the cell surface by HLA molecules (PubMed 34475551). Because of its critical role in autonomic function, therapeutic strategies must carefully avoid off-target effects in the central nervous system.

Mechanism of action

PHOX2B functions as a transcription factor that regulates the expression of genes essential for autonomic neuron development, such as tyrosine hydroxylase (TH) and dopamine beta-hydroxylase (DBH) (UniProt P54871). Therapeutic approaches under development, such as CAR-T cells and TCR-like antibodies, target PHOX2B-derived peptides presented by Major Histocompatibility Complex (MHC) molecules on the surface of tumor cells (PubMed 34475551).
