Overview

Pancreatic alpha-amylase (AMY2A) is a major digestive enzyme synthesized by pancreatic acinar cells and secreted into the duodenum to facilitate the breakdown of complex carbohydrates [1, 6, 14]. It functions as an endohydrolase, specifically catalyzing the hydrolysis of internal alpha-1,4-glycosidic linkages in starch and glycogen to produce maltose, maltotriose, and alpha-limit dextrins [11, 15, 20]. This enzymatic activity is the initial and rate-limiting step in the digestion of dietary polysaccharides, making it a critical determinant of the rate at which glucose enters the bloodstream [8, 14]. In clinical medicine, elevated levels of pancreatic alpha-amylase in the serum or urine are used as primary biomarkers for diagnosing acute pancreatitis, while abnormally low levels can indicate chronic pancreatitis or exocrine pancreatic insufficiency [4, 5, 12]. Therapeutically, the enzyme is a key target for managing type 2 diabetes and obesity; inhibitors such as acarbose and miglitol are used to delay starch digestion, thereby flattening postprandial blood glucose spikes [7, 9, 16]. However, the inhibition of this enzyme can lead to gastrointestinal side effects, such as flatulence and diarrhea, as undigested carbohydrates undergo bacterial fermentation in the colon [16, 21].

Mechanism of action

Inhibition of the hydrolysis of alpha-1,4-glycosidic bonds in starch, thereby slowing the production of absorbable sugars and reducing postprandial blood glucose elevations [8, 9, 14, 15, 16].
