Overview

Parathyroid hormone type 1 receptor (PTH1R) is a class B G protein-coupled receptor that serves as the primary receptor for both parathyroid hormone (PTH) and parathyroid hormone-related protein (PTHrP). It plays a central role in regulating blood calcium levels through actions in bone and kidney tissues. In bone, PTH1R stimulates osteoblasts to express RANKL, increasing bone resorption. In the kidney, it promotes calcium reabsorption and phosphate excretion. Mutations in *PTH1R* are associated with skeletal disorders. Agonists are used for osteoporosis management. Recent structural studies have clarified ligand binding and activation mechanisms, providing templates for drug design.

Agonists stimulate adenylyl cyclase via Gs, leading to increased cAMP production; can also activate phospholipase C via Gq; may have prolonged endosomal signaling.
