Pathobionts are commensal microorganisms that typically reside harmoniously within the host's microbiota but possess the potential to induce pathology under conditions of environmental or host perturbation, such as immunodeficiency or dysbiosis (Round & Mazmanian, 2009, Nature Reviews Immunology). Unlike primary pathogens, which are not typically members of the normal flora, pathobionts are indigenous organisms that only transition to a virulent state when the ecological balance of the microbiome is disrupted (Chow et al., 2011, Current Opinion in Immunology). They are increasingly recognized as critical drivers in the etiology of chronic inflammatory diseases, including inflammatory bowel disease, where they can trigger aberrant Th17 or Th1 immune responses (Horowitz et al., 2015, Environmental Microbiology). Therapeutic targeting of pathobionts is a complex challenge, as treatments must ideally be selective enough to eliminate the pathogenic activity without eradicating the protective commensal populations that maintain host health. Current research focuses on precision microbiome editing, including the use of narrow-spectrum antibiotics, bacteriophages, and engineered probiotics to modulate these populations (Adak & Khan, 2019, Journal of Cellular Physiology).
Pharmacological interventions target pathobionts through selective antimicrobial activity (inhibiting cell wall synthesis or DNA replication), competitive exclusion via live biotherapeutic products, or targeted neutralization of virulence factors using monoclonal antibodies and bacteriophages to restore microbial eubiosis.
