Overview

Pathogenic gut and vaginal microorganisms refer to a diverse array of bacteria, fungi, and protozoa that cause disease within the gastrointestinal and urogenital tracts. In the gut, pathogens such as Clostridioides difficile and Salmonella enterica utilize virulence factors to cause inflammation, diarrhea, and epithelial damage (StatPearls, 2023). In the vaginal environment, species like Gardnerella vaginalis and Candida albicans can outcompete beneficial Lactobacillus, leading to dysbiosis, increased pH, and the formation of resilient biofilms (NIH, 2022). These microorganisms are typically targeted by antimicrobial drugs that disrupt essential biological processes like cell wall synthesis or DNA replication (PubMed, 2021). However, because this term encompasses a wide variety of distinct species rather than a specific molecular target like a receptor or enzyme, it is classified as a clinical or ecological category. Therapeutic challenges include the development of antimicrobial resistance and the unintended depletion of the healthy commensal microbiome (CDC, 2022).

Mechanism of action

Antimicrobial agents target these microorganisms through various mechanisms, including the inhibition of DNA synthesis (e.g., metronidazole), inhibition of cell wall peptidoglycan synthesis (e.g., vancomycin), inhibition of protein synthesis via ribosomal binding (e.g., clindamycin), and disruption of fungal cell membrane integrity (e.g., fluconazole).
