Pathologic neovascular endothelial cell membranes refer to the specialized surface of endothelial cells forming abnormal new blood vessels in conditions like cancer and age-related macular degeneration (AMD) (Gerber et al., 2011, Clin Cancer Res). These membranes are molecularly distinct from normal vasculature, notably exhibiting the externalization of phosphatidylserine (PS) to the outer leaflet, which serves as a docking site for therapeutic antibodies like bavituximab (Bonderman et al., 2007, Thromb Haemost). They also overexpress markers such as integrin alpha-v beta-3 and VEGFR2, facilitating targeted delivery of vascular disrupting agents (VDAs) and photosensitizers (Tozer et al., 2005, Nat Rev Cancer). Drugs like verteporfin selectively accumulate in these membranes, allowing for localized destruction via photodynamic therapy (Schmidt-Erfurth et al., 1999, Ophthalmology). By targeting these unique membrane features, therapies can selectively occlude or destroy diseased vessels while minimizing damage to healthy, quiescent vasculature (Thorpe, 2004, Clin Cancer Res).
Therapeutic agents target these membranes by binding to exposed anionic phospholipids, such as phosphatidylserine, to trigger immune-mediated destruction (e.g., ADCC), or by selectively accumulating in neovascular cells to induce vascular collapse through microtubule disruption or photochemical damage (Thorpe, 2004, Clin Cancer Res; Schmidt-Erfurth et al., 1999, Ophthalmology).
