Pathologic neovascular endothelium refers to the specialized, rapidly proliferating endothelial cells that form new, disorganized, and hyperpermeable blood vessels in response to disease-driven angiogenic stimuli. Unlike the quiescent endothelium of healthy adult vasculature, these cells express distinct molecular markers such as vascular endothelial growth factor receptors (VEGFRs), integrin alpha-v beta-3, and endoglin (CD105) (PMID: 15549031). This cellular target is critical in the progression of solid tumors, where it facilitates nutrient delivery and metabolic waste removal, and in exudative ocular diseases like wet age-related macular degeneration, where it causes fluid leakage and hemorrhage (NIH, StatPearls). Therapeutic strategies targeting this endothelium include anti-angiogenic agents that block growth factor signaling and vascular disrupting agents (VDAs) that induce the rapid collapse of the existing pathologic vessel cytoskeleton (PMID: 15155305). Because these vessels are structurally fragile and functionally distinct from healthy vasculature, they provide a unique opportunity for selective therapeutic intervention, although systemic inhibition can lead to side effects such as hypertension and impaired wound healing.
