Overview

The patient-specific tumor B-cell receptor idiotype is a unique antigenic determinant formed by the variable region of the immunoglobulin (Ig) displayed on the surface of all malignant B cells in a given patient with a B-cell lymphoma or leukemia.[2][3][4] This idiotype arises from the unique V(D)J recombination events in each malignancy, making it a clonal, tumor-specific marker distinct to each patient.[2][3] Because it is not shared between patients, therapeutic strategies such as monoclonal anti-idiotype antibodies and idiotype vaccines must be tailored for each individual.[1][3] Targeting the idiotype enables specific recognition and destruction of tumor cells by the immune system, either via direct antibody-mediated mechanisms or by stimulating host immune responses through vaccination.[1][3][4] The idiotype is used as a biomarker for monitoring disease progression and therapeutic response, with its serum levels correlating with tumor burden.[1]
