A **patient-specific tumor idiotype** refers to the unique antigenic determinants (idiotypes) found within the variable regions of the immunoglobulin molecules produced by a malignant B-cell clone, such as in B-cell lymphomas or multiple myeloma[1][2][3][5]. Because these idiotypes are unique to each patient’s tumor and expressed on the surface of tumor cells, they serve as highly specific tumor-associated antigens and targets for individualized immunotherapy, including vaccines and monoclonal antibodies. Immunization strategies stimulate a patient’s immune system to mount responses specifically against the malignant cells, leaving normal cells unharmed. This approach has shown promise in clinical trials, although challenges remain in consistently eliciting strong immune responses due to the low inherent immunogenicity of self-idiotypes and the immunosuppressive tumor microenvironment[1][2][3].
Vaccination: Induces immune response (mainly T-cell and/or antibody response) specifically targeting malignant B cells expressing the idiotype[1][2][3][5][7]. Monoclonal antibody: Binds idiotype on tumor cell, can mediate cytotoxicity or mark cell for immune clearance[1][2][3]. Radioimmunotherapy: Delivers radiation specifically to idiotype-expressing tumor cells via radiolabeled antibody or sdAb[2]. Dendritic cell vaccination: Presents idiotype to immune system for activation of anti-tumor immunity[3][7].
