Patient-specific tumor neoantigens are unique peptides resulting from somatic mutations within a patient's tumor that are not found in healthy tissue (Schumacher & Schreiber, 2015). These neoantigens are processed and presented on the surface of cancer cells by Major Histocompatibility Complex (MHC) molecules, forming a specific NeoAg-MHC complex (Janeway et al., 2001). The cognate T-cell receptor (TCR) is the specialized molecule on T-lymphocytes that recognizes this complex, initiating a targeted immune attack against the malignant cells (Rosenberg & Restifo, 2015). Because these targets are truly tumor-specific, they offer a high degree of therapeutic selectivity, minimizing the risk of damage to normal organs (Blass & Ott, 2021). Current therapeutic modalities targeting this interaction include personalized mRNA or DNA vaccines designed to stimulate the patient's own T-cells, as well as adoptive cell therapies where T-cells are engineered to express a TCR specific to the patient's unique neoantigens (Sahin & Türeci, 2018). This target paradigm represents a cornerstone of precision oncology, shifting treatment from generic agents to highly individualized immunotherapies tailored to the unique mutational landscape of each patient (Ott et al., 2017).
