Overview

Peanut-specific immunoglobulin E antibody is a subtype of IgE antibody generated by B cells that recognizes one or more protein components in peanuts (most commonly Ara h 1, Ara h 2, Ara h 3, Ara h 6, Ara h 8, and Ara h 9). These antibodies bind to peanut proteins, crosslinking FcεRI receptors on mast cells and basophils, leading to cell activation and release of mediators such as histamine, which triggers the symptoms of allergic reactions and anaphylaxis. Peanut-specific IgE is used clinically as a biomarker to diagnose and assess the risk/severity of peanut allergy through serum immunoassays and component-resolved diagnostics. Therapeutic approaches targeting IgE include monoclonal antibodies like omalizumab, which block IgE from binding to its receptor and thereby prevent cell activation and allergic symptoms. Safety concerns exist due to the potential for life-threatening anaphylactic reactions in sensitized individuals. Cross-reactivity between peanut allergens and allergens from other sources, such as birch pollen (Ara h 8/Bet v 1), adds complexity to diagnosis and management.
