Perilipin-4 (PLIN4) is a large, repetitive protein predominantly found coating the surface of lipid droplets in adipocytes and skeletal muscle. Its amphipathic helical region is exceptionally long, enabling PLIN4 to form a stable, nearly immobile coat that regulates lipid droplet size and protects droplet contents from degradation. PLIN4 selectively associates with droplets rich in cholesterol esters and may participate in lipid droplet biogenesis. Mutations that result in repeat expansion in PLIN4 are linked to rare cases of autosomal-dominant muscular dystrophy with rimmed vacuoles. In adipose tissue and muscle, PLIN4 concentration influences lipid droplet characteristics and responds to metabolic cues such as physical activity. Although implicated in specific pathologies and metabolic regulation, PLIN4 is not currently considered a direct therapeutic drug target[1][2].
