Overview

Peripheral sensory nerves in the upper calf refer to the anatomical collection of cutaneous nerves, primarily the saphenous and sural nerves, that provide sensory innervation to the skin of the lower leg (StatPearls, 2023). These nerves are responsible for transmitting afferent signals, including touch, temperature, and nociceptive (pain) information, from the periphery to the central nervous system (NIH, 2022). While the term describes an anatomical region rather than a specific molecular target, these nerves contain various ion channels and receptors that are critical for signal transduction. They are the primary site of action for local anesthetics, which block voltage-gated sodium channels to provide regional anesthesia or analgesia (PubMed, 2021). Dysfunction of these nerves is a hallmark of peripheral neuropathies, often resulting from metabolic conditions like diabetes or physical compression, leading to chronic pain or sensory deficits (Mayo Clinic, 2023). Therapeutic strategies targeting the components of these nerves aim to alleviate neuropathic pain or provide temporary sensory blockade for surgical procedures (StatPearls, 2023).

Mechanism of action

Local anesthetics interact with these nerves by binding to and inhibiting voltage-gated sodium channels (e.g., Nav1.7, Nav1.8) on the axonal membrane, which prevents the depolarization necessary for action potential propagation and signal transmission (PubMed, 2021). Neuropathic agents like gabapentinoids modulate the alpha-2-delta subunit of voltage-gated calcium channels to reduce the release of excitatory neurotransmitters at the synapse (StatPearls, 2023).
