Phosphate anions are essential inorganic ions required for a multitude of biological processes, including the formation of hydroxyapatite in bone, the production of cellular energy via ATP, and the structural integrity of nucleic acids and cell membranes [PubChem, 2024]. In healthy individuals, phosphate levels are tightly regulated by the kidneys through filtration and reabsorption; however, in patients with advanced chronic kidney disease (CKD), renal excretion is severely impaired, leading to hyperphosphatemia [NIH, 2023]. Elevated serum phosphate is a primary driver of secondary hyperparathyroidism and systemic vascular calcification, which significantly increases cardiovascular risk in renal patients [Kidney International, 2017]. Pharmacological management focuses on reducing the absorption of dietary phosphate using phosphate binders, which are non-absorbable agents that sequester phosphate within the gastrointestinal tract [StatPearls, 2023]. These drugs are critical for managing mineral and bone disorders and are a cornerstone of therapy for patients on dialysis.
Phosphate binders act by chelating or binding to dietary phosphate in the gastrointestinal tract, forming insoluble complexes that are excreted in the feces, thereby reducing systemic absorption [StatPearls, 2023; NIH, 2022].
