Phrenic nerve activity (PNA) refers to the rhythmic neural impulses originating from the brainstem's respiratory centers that stimulate the diaphragm to initiate inhalation (StatPearls: NBK536934). It is primarily generated by the pre-Bötzinger complex in the medulla and transmitted through phrenic motor neurons located in the cervical spinal cord (C3-C5). While PNA is a physiological measurement of respiratory drive rather than a discrete molecular target like a receptor or enzyme, it is a fundamental endpoint in pharmacological research used to assess the respiratory effects of various compounds. For example, opioid analgesics are known to dangerously suppress PNA, which can lead to fatal respiratory depression, whereas respiratory stimulants such as ampakines are investigated for their ability to enhance this activity (PubMed: 21917105; PubMed: 16835234). Monitoring and understanding PNA is essential for managing conditions such as central sleep apnea, spinal cord injury, and neurodegenerative diseases like ALS that compromise respiratory function.
Drugs do not target phrenic nerve activity directly; instead, they modulate it by acting on molecular targets such as mu-opioid receptors (MOR), GABA receptors, or AMPA receptors within the brainstem respiratory rhythm generators (e.g., pre-Bötzinger complex) and phrenic motor neurons (PubMed: 21917105; PubMed: 16835234).
