Overview

The physiological coagulation cascade is a complex series of proenzyme-to-enzyme conversions that culminate in the formation of a stable fibrin clot to prevent blood loss following vascular injury (StatPearls, 2023). It is traditionally divided into the intrinsic (contact activation) and extrinsic (tissue factor) pathways, which converge on the common pathway to activate thrombin (NIH, 2022). Thrombin then converts soluble fibrinogen into insoluble fibrin strands, which are cross-linked to stabilize the clot (PubMed, 2021). While essential for hemostasis, dysregulation of this cascade can lead to pathological thrombosis or life-threatening hemorrhage (Wikipedia, 2024). Pharmacological intervention typically targets specific serine proteases within the cascade, such as Factor Xa or Thrombin, to treat or prevent thromboembolic disorders (PubChem, 2023). This system is a critical focus for anticoagulant therapy in conditions like atrial fibrillation and deep vein thrombosis.

Mechanism of action

Drugs targeting the coagulation cascade act by inhibiting specific clotting factors (e.g., Factor Xa inhibitors like Rivaroxaban, Direct Thrombin Inhibitors like Dabigatran), accelerating natural inhibitors (e.g., Heparin via Antithrombin III), or depleting functional clotting factors (e.g., Warfarin via Vitamin K antagonism) (StatPearls, 2023).
