Overview

The physiological coagulation cascade and plasma oncotic pressure are two distinct but interrelated systems essential for vascular integrity and fluid homeostasis. The coagulation cascade consists of a series of proteolytic activations of clotting factors that culminate in fibrin formation to stop bleeding (StatPearls, 2023). Plasma oncotic pressure is the osmotic pressure exerted by proteins, primarily albumin, which maintains blood volume by pulling water into the capillaries (NIH, 2022). Disorders in these systems can lead to thrombosis, hemorrhage, or systemic edema. Pharmacological agents target these systems either by inhibiting specific enzymes in the clotting cascade to treat thromboembolic diseases or by providing protein-rich solutions to restore oncotic pressure in conditions like hypoalbuminemia or shock.
