Pituitary corticotrophs are specialized endocrine cells located in the anterior pituitary gland that play a central role in the hypothalamic-pituitary-adrenal (HPA) axis (StatPearls, 2023). Their primary biological function is the synthesis and secretion of adrenocorticotropic hormone (ACTH), which is cleaved from the precursor polypeptide pro-opiomelanocortin (POMC) (NCBI, 2022). The activity of these cells is primarily stimulated by corticotropin-releasing hormone (CRH) and arginine vasopressin (AVP) and is inhibited by the negative feedback of glucocorticoids like cortisol (StatPearls, 2023). In clinical medicine, the most significant pathology involving these cells is Cushing's disease, where a corticotroph adenoma hypersecretes ACTH, leading to systemic hypercortisolism (NIDDK, 2024). Therapeutic interventions for corticotroph-related disorders often involve pharmacological agents that target specific receptors on the cell surface, such as somatostatin receptor subtype 5 (SSTR5) or dopamine D2 receptors (Journal of Clinical Endocrinology & Metabolism, 2012). For example, the somatostatin analog pasireotide is used to inhibit ACTH secretion in patients with Cushing's disease by binding to SSTR5 (FDA, 2012). While these cells are the physiological source of ACTH, they represent a cell population rather than a single molecular target, making them a cellular site of action for various ligands.
Drugs typically act as agonists at inhibitory receptors (e.g., SSTR5, D2) on the corticotroph cell surface to reduce ACTH secretion and potentially inhibit cell proliferation in adenomas.
