Overview

The plantar fascia is a dense, fibrous band of connective tissue, also known as the plantar aponeurosis, extending from the medial tubercle of the calcaneus to the proximal phalanges of the toes [1]. Its primary biological function is to provide structural support for the medial longitudinal arch of the foot and to act as a shock absorber through the windlass mechanism during gait [1, 2]. Pathological conditions such as plantar fasciitis involve degenerative changes and inflammation of this tissue, often resulting from repetitive strain or overuse [2, 3]. While it is not a molecular target in the traditional sense (like a receptor or enzyme), it is a focal point for therapeutic interventions including corticosteroid injections and NSAIDs aimed at reducing localized inflammation [1, 3]. Chronic mismanagement or excessive steroid use can lead to complications such as fascial rupture or atrophy of the surrounding fat pad [1].

Mechanism of action

Drugs typically target the inflammatory processes or pain signaling associated with the tissue rather than the tissue itself as a receptor. Corticosteroids reduce inflammation by inhibiting phospholipase A2 and reducing prostaglandin synthesis [1]. NSAIDs inhibit cyclooxygenase (COX) enzymes to provide analgesic and anti-inflammatory effects [3]. Botulinum toxin type A may be used to modulate pain pathways or reduce muscle tension in the kinetic chain [2].
