The Plasmodium hypnozoite liver stage form refers to a dormant intracellular stage of malaria parasites (Plasmodium vivax, Plasmodium ovale, and some simian species) that persists in the liver of infected individuals after initial exposure. Unlike replicating liver stages that give rise directly to blood-stage parasites upon first infection, hypnozoites remain metabolically quiescent for weeks to years, eventually "awakening" to cause recurrent episodes of malaria (relapses)[4][3][2][7]. Hypnozoite biology is poorly understood because it is not a single molecular entity but a developmental state; current medical interventions are limited to drugs that directly or indirectly disrupt its survival or activation (primarily the 8-aminoquinolines). The presence of hypnozoites underlies the unique relapsing pattern of P. vivax and P. ovale malaria, complicating eradication and control efforts due to the instability of latency and the emergence of drug resistance. Laboratory identification relies on stage-specific markers (e.g., LISP2, UIS4), but clinical detection tools are still lacking, representing a major challenge for research and elimination strategies[1][3][6].
