Overview

Hypnozoites are dormant liver-stage forms of certain Plasmodium species (notably *P. vivax* and *P. ovale*) that persist in hepatocytes and cause relapsing malaria. Primaquine and tafenoquine are currently the only drugs clinically active against this stage and are essential for "radical cure," though their use is limited by safety concerns. Gametocytes are the sexual forms of Plasmodium species in the blood, responsible for human-to-mosquito transmission and are also susceptible to primaquine. Both stages represent cellular forms of the parasite rather than single molecular targets, making them challenging for drug discovery and structured target annotation. The distinction between hypnozoite and gametocyte stages is critical for accurate research and pharmacological classification.

Mechanism of action

Primaquine metabolites generate reactive oxygen species and disrupt parasite mitochondria; their efficacy depends on host metabolism (CYP2D6). Tafenoquine has a related mechanism, acting on dormant liver forms.
