Overview

The platelet activation and aggregation machinery represents the integrated system of cell surface receptors, intracellular signaling pathways, and effector proteins that govern the platelet response to vascular injury [1]. This machinery includes critical therapeutic targets such as the P2Y12 ADP receptor, the protease-activated receptor 1 (PAR-1), and the glycoprotein IIb/IIIa integrin complex [2]. Under physiological conditions, this system ensures hemostasis by forming a platelet plug; however, its overactivation leads to arterial thrombosis, a primary cause of myocardial infarction and ischemic stroke [3]. Antiplatelet drugs target specific nodes within this machinery, such as COX-1 (inhibited by aspirin) or P2Y12 (inhibited by clopidogrel), to reduce the risk of thrombotic events [4]. The therapeutic management of this machinery is complex, as inhibiting these pathways inherently increases the risk of bleeding complications [5]. Consequently, monitoring platelet function through various biomarkers is often employed to optimize treatment efficacy and safety [6].

Mechanism of action

Inhibition of various components including cyclooxygenase-1 (COX-1) to prevent thromboxane A2 synthesis, antagonism of P2Y12 purinergic receptors to block ADP-induced activation, blockade of glycoprotein IIb/IIIa receptors to prevent fibrinogen cross-linking, and antagonism of protease-activated receptor-1 (PAR-1) to inhibit thrombin-mediated signaling.
