Overview

The Platelet factor 4 (PF4) – ChAdOx1 adenoviral capsid complex is a pathological molecular assembly formed by the electrostatic binding of the positively charged chemokine PF4 to the negatively charged hexon proteins of the ChAdOx1 viral vector (Baker et al., 2021, Science Advances). This complex acts as a neoantigen, inducing the production of high-affinity IgG autoantibodies in susceptible individuals (Greinacher et al., 2021, NEJM). These antibodies bind to the complex and subsequently activate platelets by cross-linking FcγRIIa receptors, leading to a prothrombotic state and platelet consumption (McGonagle et al., 2021, Lancet Rheumatology). This mechanism is the hallmark of Vaccine-induced Immune Thrombotic Thrombocytopenia (VITT), a rare but serious adverse event associated with adenoviral vector-based COVID-19 vaccines. Clinically, the presence of this complex and the resulting antibodies leads to severe venous and arterial thrombosis, often in atypical locations such as the cerebral venous sinuses. Management involves the use of non-heparin anticoagulants and high-dose intravenous immunoglobulin to block platelet activation. Research into this complex is vital for designing modified adenoviral vectors with reduced surface electronegativity to prevent such interactions in future therapeutics (Kowarz et al., 2021).
