The posterior mitral valve leaflet is one of two primary leaflets of the mitral valve, located between the left atrium and left ventricle. It typically has a quadrangular or semi-oval shape and comprises about two-thirds of the circumferential attachment to the mitral annulus. The leaflet is thinner, shorter, and more flexible than the anterior leaflet and is divided by two clefts into three scallops (P1, P2, P3), which are clinically relevant for surgical and echocardiographic localization. The posterior leaflet plays a critical role in valve coaptation and, when dysfunctional (such as in prolapse or annular dilation), may contribute to mitral regurgitation. Unlike protein or receptor drug targets, the "posterior mitral valve leaflet" is an anatomical structure and not a molecular or pharmacological target
