The posterior tibial nerve is a major peripheral nerve of the lower limb, representing the distal continuation of the tibial nerve as it passes through the tarsal tunnel behind the medial malleolus [5, 12]. It provides motor innervation to the intrinsic muscles of the foot and sensory innervation to the plantar surface and heel [4, 10]. While the nerve itself is an anatomical structure and not a specific molecular target like a protein or receptor, it serves as a critical focus for medical interventions including regional anesthesia and neuromodulation [8, 11]. Specifically, percutaneous tibial nerve stimulation (PTNS) is an established therapeutic approach for managing overactive bladder and fecal incontinence by modulating spinal reflexes [3, 7]. Pathological conditions associated with the nerve include tarsal tunnel syndrome, where nerve compression leads to pain and sensory loss, and diabetic neuropathy, which can cause significant nerve swelling detectable by ultrasound [6, 17]. Pharmacological interaction is primarily achieved through local anesthetics, which block sodium channels in the nerve fibers to induce regional analgesia [8].
Local anesthetics like lidocaine interact with the posterior tibial nerve by blocking voltage-gated sodium channels on the axonal membrane, which inhibits the ionic fluxes necessary for the initiation and conduction of nerve impulses [8]. In clinical procedures such as percutaneous tibial nerve stimulation (PTNS), electrical modulation of the nerve sends afferent signals to the sacral micturition center to inhibit overactive detrusor contractions [7, 13].
