Povidone-iodine is a stable chemical complex of polyvinylpyrrolidone (PVP) and elemental iodine, primarily used as a broad-spectrum antiseptic and disinfectant (PubChem). It is not a biological target such as a receptor or enzyme; rather, it is a therapeutic agent that delivers iodine to the site of action. The complex acts as a reservoir, slowly releasing free iodine which exerts rapid microbicidal activity against bacteria, viruses, fungi, and protozoa (StatPearls). Its mechanism involves the non-specific oxidation of essential microbial components, including proteins and nucleic acids, which prevents the development of resistance. Povidone-iodine is extensively used in clinical settings for preoperative skin preparation, wound care, and the treatment of minor cuts and burns (NIH). While generally safe for topical use, systemic absorption can occur, particularly in large wounds, potentially leading to thyroid dysfunction or electrolyte imbalances.
Povidone-iodine acts as an iodophor, providing a sustained release of free iodine. The free iodine penetrates microbial cell walls and membranes, where it oxidizes key biological molecules including sulfur-containing amino acids, nucleotides, and fatty acids, resulting in the denaturation of proteins and disruption of cellular metabolism and structure (StatPearls).
